my question is: could I have failed the b.c. because of bad resume dates?

Most times, we don't call references, because they're friends who will say good things about you.
Usually I don't tell the candidate that they failed the background check – if they call I tell them that they're not hired, and if they press I may tell them – we're not requried by law to notify you if you fail the b.c.
and yes, lying on your resume is a reason to fail – if you're off by a couple of months, that's ok, when you're off by years, well, that's a problem.

Resumes mean nothing, it is the application that counts.
No, as there are many more applicants than jobs, HR rarely calls to tell people that they are not being hired. It is never a good phone call and when you are already swamped it is easy to never get around to the uncomfortable part of your job.
If you lied about your background odds are the hiring process stopped right there. Don't people realize that our economy is tanking and your competition is fierce? Probably not.
As to personal references, they are rarely called. Only the dumbest of the dumb would ever list a reference of someone who doesn't like them (though it has happened).
Calm down, there may still be hope. Give them a few days leeway before you totally freak out. But if you are doing the one interview and wait game it will take you a long time to find a job. It is a numbers game, keep searching and interviewing until you accept an offer, not until you get an interview.
